By signing up, I understand and agree that Box Wellness Co. education + exercise programs are not medical diagnosis or treatment. I understand and acknowledge that information provided by Box Wellness Co. through educational course, on the Box Wellness Co. website, the Box Wellness Co. Instagram or Facebook Page, or other Box Wellness Co. platforms, are for informational purposes only. This information is not a substitute for professional medical advice, diagnosis, or treatment.
By signing up, I understand and agree that Box Wellness Co. programs do not establish a therapist-patient relationship between course facilitators and participants. I agree that I am participating in Box Wellness Co. services solely for my personal knowledge, education and awareness. By signing up, I acknowledge that there is no guarantee or warranty that the program will lead to any specific goal or particular results and Box Wellness Co. makes no promise that each Client will experience the same or similar results. I acknowledge that the Services and materials provided are provided without any express or implied warranties of any kind.
By signing up, I release, indemnify and save harmless Box Wellness Co., its directors, officers, agents, employees, contractors, volunteers, heirs, executors, administrators, successors, and assigns, as applicable (collectively, “Released Parties”) from any and all liability and damages arising from the Services, including financial damages or personal injuries, however caused, including negligence, during my engagement of the Program or Services. I further agree to forfeit all forms of legal recourse which may be available to me, including but not limited to any form of damages, costs, losses or expenses as a result of the Services. I acknowledge this release of liability is binding on my heirs, executors and anyone else who may be able to bring a legal action on my behalf in the future. This clause survives the expiration or early termination of this Agreement.
By signing up, I acknowledge there are certain risks associated with participating in the Program or Services, including, but not limited to, financial, physical, emotional, spiritual loss and damages, and I voluntarily assume such risks. Risks from the coaching techniques may include dizziness, fainting, physical discomfort, and any other pain resulting from the Services, whether or not the symptoms came as a result of my negligence, negligence of others, or negligence of Box Wellness Co. Should I have any concerns or doubts about ability to participate, I will contact Box Wellness Co. immediately to inform them of any concerns and seek the appropriate professional medical advice as needed. I confirm that I have full capacity and am the age of majority in my jurisdiction of residence.
